About Forest View

Forest View Center welcomes you to our rehabilitation and residential health care center, located in Forest Hills, New York. We are a licensed skilled nursing facility, specializing in aggressive rehabilitation for both short-term and long-term patients. We feature two dedicated short-term rehabilitation units and offer treatment for patients who require sub-acute, respiratory, and rehabilitative services.

We believe that optimism and positivity are the cornerstones of healing. We stay committed to providing a warm, caring atmosphere for our residents, patients, and visitors. Our goal is to foster an environment that not only delivers professional, quality care but also incorporates many touches of home. We strive to deliver innovative and progressive healthcare and aim to create a comfortable, enjoyable environment that offers both a sense of home and community.  The amenities we provide are designed to make a stay at Forest View both relaxing and convenient - allowing you to focus on improving your health and well-being.

We invite you to visit our Admissions and Social Services departments. To facilitate a worry-free transfer experience to our facility, Forest View is proud to collaborate with all major metropolitan area hospitals. We provide referrals, help you inquire about insurance eligibility, and offer Medicaid counseling services to assist you and your loved one in choosing the best options of care.

All public spaces and patient/resident rooms at Forest View are well lit, invitingly furnished, and tastefully decorated. For comfort and privacy, patient/resident rooms have fully automated electric beds, cable televisions, phone service, and on-suite bathrooms. As an added convenience, we offer immaculate, bright dining rooms on each level of our facility – including a large central dining room, located in our penthouse. Each dining suite is decorated with beautiful natural decor and equipped with cable television. Our amenities include:

Our Staff

Our professional, certified staff features caring physicians, psychiatrists, psychologists, physical, occupational, respiratory, and speech therapists, nurses, social workers, skilled dietary staff, and nutritionists. These dedicated team members, along with our recreational therapy department, help meet the comprehensive needs of our clients.

Through continuing education, our staff members stay informed of the newest care techniques and most up-to-date therapy methods, ensuring that our patients receive outstanding, high-quality care.

Our Physicians

All of our attending physicians specialize in internal and geriatric medicine and are both in our facility daily and on call 24-hours a day. Our primary care doctors and consultants are affiliated with many major hospitals in our area, such as New York Hospital of Queens, St. John's, (North Shore LIJ) Forest Hills, Parkway, Mary Immaculate, and Long Island Jewish Hospital, and provide ongoing, on-site medical services and when our patients are under hospital care.

Additional attending physicians include specialists in gastroenterology, neurology, orthopedics, pulmonology, ophthalmology, psychiatry, pain management, and vascular disease.

Social Services

Have a question or need a helping hand? Our experienced social workers offer patients and families the assistance and emotional reassurance they need to ease the transition into our facility. Psychological counseling services are also available for additional support.

Social workers regularly visit our patients to address each individual’s feelings and concerns. They also stay in close contact with family members to make sure that loved ones are well informed. Social workers communicate with families on a wide variety of topics, including insurance and financial counseling, resident progress, facility practice, and referrals as well as play active roles in grief counseling and discharge planning.

Volunteers & Community

A variety of outreach initiatives foster a close link to our community, and volunteers enrich our residents’ recovery through visits and recreational programs. We maintain many close connections with local organizations and houses of worship throughout our neighboring communities.

Admissions Process

Comprehensive Care Planning

When a patient is admitted to Forest View, the patient and/or the family meets with a team of therapists, nurses, and social workers to develop their individualized Comprehensive Care Plan, which coordinates and guides all of the facets of each patient's treatment throughout their stay.

Using an interdisciplinary approach, our Continuing Care Program continues to make sure that each individual receives the personalized care he or she needs and meets their goals of healing and recovery.

Discharge planning is part of this process, and begins prior to the patient’s arrival at Forest View, We make sure that when discharged, each patient is educated and fully prepared for the level of care he or she will be receiving.

We also work with insurance companies and medical equipment suppliers to help patients get the Durable Medical Equipment they will need when they get home.

Payment Methods

We accept private-pay and participate in all third-party payment programs including Medicare and Medicaid, and participate with most major managed care organizations and HMO’s.

Arrangements can always be made with any HMO at your request, subject to the HMO eligibility requirements. Our financial assistance office will gladly consult with potential applicants, and assist with the preparation and submission of HMO and Medicaid applications.

We assist in obtaining HMO approval, maintaining approval, sending updates to the HMO, and in general, dealing with all aspects of the HMO.

Help with Medicaid/ Medicare Applications

We accept pending Medicaid patients, and we assist with the preparation and submission of Medicaid applications.

How to get Here

We are conveniently located right off Queens Boulevard, in Forest Hills, N.Y. and accessible by all forms of public transportation, as well as close to all the major highways. Our close proximity to all area hospitals allows for almost immediate access to critical care when needed. We are located in a safe residential area with on and off street parking available.

Subway:
Independent Sixth Avenue F or Eighth Avenue E train to Continental and 71st Avenue station. Walk one block north of Queens Blvd. Long Island Railroad to Forest Hills station. Proceed 3 blocks north.

Train (LIRR):
Long Island Railroad to Forest Hills station. Proceed 3 blocks north.

Bus:
Queens Blvd. bus Q-60 to Continental Avenue. Walk one block north of Queens Blvd.
